At no stage will the child's name be shown alongside the photograph I give permission for my child/children to be photographed and/or videoed during any JSD class or event.
I give permission for my child/children to be professionally photographed and/or videoed during live stage productions and rehearsals.
I give permission for these photographs and videos to be used for marketing purposes, including programmes, posters, banners, JSD website, Instagram and Facebook pages.

Health

I understand the nature of physical activity involved in dance and musical theatre and certify that my child is medically fit to participate in these activities. If this situation changes I will inform JSD immediately.
